IF YOU ENCOUNTER PROBLEMS RENEWING ONLINE, print out and complete the renewal form (see below) and fax it to the MPSA National Office (1-812-335-1510). IF YOU FORGOT YOUR PASSWORD AND YOU DO NOT RECEIVE AN AUTOMATIC E-MAIL RESPONSE, request your password by e-mail at mpsainfo@mpsanet.org or by phone at 1-812-558-0588, extension 0, 701, or 705 during office hours (8:00 a.m. to 5:00 p.m., EDT).

If you prefer not to join or renew using our secure server, please print a Membership Application/Renewal Form and mail or fax it to us. Our mailing address and fax number are provided on the form. Two different file formats are available, Rich Text File or RTF, which can be opened by your word processing program, and PDF, which can be opened using Adobe Acrobat Reader.
